- Non-magical item purchasable from vendors (eg. Dagger)
- Normal Monster
- Non-magical item not from vendors, only dropped by monsters (eg. Boarman's Cuirass from Boarman)
- Magical
- Normal item with up to 2 magical affixes; prefix and/or suffix (eg. Resistant Ring of Mana)
- Magical Monster
- Monster dropped item with up to 2 magical affixes; prefix and/or suffix. Affixes usually not named (eg. Wraith's Cage with stats from unnamed Thorns affix from Drowned Sailor)
- Rare
- Magical item with up to 2 special Rare affix (see Galefury's Complete Rare Affix List). can have a combination of rare and magical prefix/suffix (eg. Veteran's Spear of Fire)
- Rare Monster
- Normal monster item, with up to 2 Rare affixes; prefix and/or suffix. Affixes usually not named (eg. Gorgon's Sash with stats from unnamed Robust affix from Gorgon)
- Monster Infrequent
- Special rare variants of Normal Monster items or unique monster-themed item. Have unique affix combinations not found on Magical or Rare items (eg. Stonebinder's Cuffs from Gorgon as unique variant of Gorgon Cuffs and Outrider's Buckler from Centaur as unique)
- Epic
- Powerful unique item with unique affixes
- Legendary
- Superior unique item with unique affixes

And yes, all white, yellow, and green items have a different power version in each difficulty. Only Epics and Legendaries are truly unique and do not change depending on difficulty. Although, some Epics and Legendaries have an unprinted variable affix (but that is another topic). And, the drop tables/selection of items does change with each difficulty (ie, you can't usually find a Legendary in Normal diff unless maybe you are a max level 65 char farming Normal Typhon).
